Appointments
We see patients by appointment.
If you are unable to keep a scheduled appointment with the doctor, we ask that you please give us 24 hours notice so that we may give your appointment to someone else in need.
Prescription Refills
For prescription refills, please contact our office by phone. Please allow 48 hours to process prescription refill requests. As a general policy, we do not request refills after hours or on weekends.
Financial / Insurance
We accept most major health insurance plans. To confirm that we accept your insurance plan, we encourage you to contact our office
to confirm.
At the time of your visit, please be prepared to pay any co-payments, deductibles or non-covered services. For your convenience, we accept cash, checks and debit/credit cards.
For your first appointment, please also bring a government issued photo ID and your insurance card if you have insurance. Please also remember to bring a list of any medications that you are currently taking, including any over the counter medications.
